Life Insurance
Life insurance provides additional financial protection for you and your family in the event of your death. The Plan provides $20,000 of life insurance for each active participant. There is no cost to you for this coverage. All members must complete a beneficiary designation form. Your beneficiary can be changed by completing a new beneficiary designation form.

Weekly Disability Income
If you’re an active Plan participant and cannot work due to a disability, the Plan provides you with financial assistance in the form of weekly disability benefits. A disability, by the Plan’s definition, is an injury or illness that prevents you from performing the duties of the job you were in right before the disabling accident, illness or injury occurred. To be eligible for benefits you must first visit a legally qualified physician and be under his or her care and provide the Fund Office with update forms every 30 days.

​Non-Occupational Disability – $250 per week for up to 26 weeks; payable from the first day of the accident or surgery or from the eighth day of illness

Occupational Disability – Up to $450 per week for up to 26 weeks; reduced by the amount of any Workers' Compensation benefit
